Your first visit to Today’s Dentistry establishes the vital foundation that allows us to build trusting relationships with our patients. From your first consultation to your final treatment, we’ll be there with you every step of the way.
During the first visit, we’ll complete a full exam of your teeth, gums, and jaw, as well as review your medical history to get a better understanding of your oral health. Dr. Burneson will also take the time to get to know you and your reason for visiting.
You can save some time at your visit by printing out and completing the patient forms below prior to your appointment.
In addition to filling out the following forms, we ask that you bring your insurance card or policy number and a list of medications you’re currently taking. The information you give by filling out these forms allows us to provide you with the best possible care.
